Determining Your Home Care Needs

Assessments


To best determine the services needed for each client, a Free in-home care needs assessment will be done by Hearts to Heal Care management staff. During the assessment, details of individual situations will be gathered allowing for us to develop a care plan that best suits the clients’ needs.  Information that will be gathered should include but is not limited to:

  • Medical history and/or medical diagnosis
  • Grooming Assistance needs (i.e. toileting, bathing)
  • Medications (for reminder purposes)
  • Nutritional Information (for dietary preparation)
  • Transportation needs (i.e. doctor’s appointments or errands)
  • Light Housekeeping needs
  • Exercise needs
